Please complete and return forms and fees to Summer Band ASAP. Forms must be completed in order to participate. If payment of fees is a hardship, please speak directly with Mr. Shaffer. The inability to pay fees will not prevent a child from participating in Band.
One band t-shirt will be provided for each band member. If you would like to order additional t-shirts, indicate on the Payment and Fees Form.
Band is considered a sport and each student must have a current Medical Physical Form on file to participate. We use the form provided by the state and it can be found at wvssac.org. The same physical form can be used for multiple sports/activities at MMS. It must be dated AFTER May 1st of the year in which the student will enter Marching Band that fall. Students are prohibited by the WVSSAC from participating without a current physical on file at MMS.
